Check-In dates for all sports are as follows:
Fall Sports – July 27th – August 11th 8am -12pm and 1-3 pm
Winter Sports – November 2-9, 2016
Spring Sports – February 15-24, 2017
Athletes planning to participate in a sport must turn in ALL REQUIRED FORMS AND FEES at check in, before the first practice. Any incomplete forms, missing signatures, and or missing payment will not be accepted and will result in your athlete not participating (practice or competition)until completed. Forms may be picked up in the main office or from your coach.

Adams 12 Authorization for Athletic Participation form
Proof of current physical (valid for one year)
Athletic Check-In Form (new one for each sport season)
Emergency Information Form(new one for each sport season)
CHSAA Student Eligibility Form
Participation Fee for 2016-2017 is $150 per sport (max $300/year) or proof of free/reduced lunch approval. Athletic Fees can be paid by cash/ check or credit card through the Bookkeeper’s office. Participation fees MUST be paid in advance at the time of check in with Mrs. Fraizer.
All paperwork must be turned into the athletic secretary – Mrs. Milani, before the season begins. Paperwork will not be accepted by your coach on the first day of practice. Please plan ahead!
